WSU student wants to complete their elective at an outside clinical site. 

Check to see if your institution is participating in VSLO

If the institution participates in VSLO, then no separate affiliation agreement is required.

If the institution does not participate in VSLO, then the student will need to complete the 4th Year Away Elective Affiliation Agreement Request form. The Scheduling coordinator recieves the form and will check to see if an affiliation agreement exists on file for the requested clinical site. After completing the formy, please be sure to complete the Independent, Research, and Non-VSLO Away Electives form as well.

Does a current affiliation agreement exist?

If a current agreement exists, then allow for 48 hours approval time. The student is allowed to complete the elective at the requested clinical site. 

If a current agreement does not exist, then a new agreement is needed.

Does the clinical site use the standard AAMC agreement?

If yes, then the agreement will take possibly 30 days. The template is sent to Dr. Steffes and the provost for signature. 
Scheduling coordinator forwards completed agreement to the clinical site. Student is then allowed to complete the elective at the requested clinical site.

If no agreement exists, the a new agreement is drafted. The drafted agreement is sent to general counsel for review, approval, and stamp. The approved agreement is sent to the clinical site for review and signature. The signed agreement is sent to Dr. Steffes and the provost for review, approval, and signature. Scheduling coordinator forwards completed agreement to the clinical site and then student is allowed to complete elective at the requested clinical site.
